Peripheral Neuropathy refers to conditions that result when nerves carrying messages from the brain and spinal cord to the rest of the body are damaged or inflamed.
When these nerves are damaged we can experience endless Burning, Numbness and Tingling, Chronic Pain and even Loss of Balance.
There are several kinds of Peripheral Neuropathies and they can stem from a variety of causes ranging from carpal tunnel syndrome (entrapment), traumatic injury (such as motor vehicle accidents, slip and fall, or sports/work related accidents), restricted blood flow, deteriorating joints, pesticide exposure, to toxic levels of sugar in the blood (causing nerve damage linked to diabetes).
At your Neuropathy appointment you will get answers to the following questions:
✅ What is the underlying cause of the nerve damage?
✅ How severe is my condition?
✅ Are other conditions contributing?
✅ What types of nerves have damage?
✅ How much treatment do nerves require to heal?

Toronto Clinical Neuropathy Scoring System (TCNS)
At Back 2 Health, we utilize the The Toronto Clinical Neuropathy Scoring System (TCNS), which is a widely used tool to assess the severity of peripheral neuropathy, particularly in individuals with diabetes or other conditions affecting peripheral nerves. It combines a clinical examination and patient-reported symptoms to provide a structured and objective evaluation of neuropathy.
Here’s an explanation of the TCNS and how it works:
The scoring system is based on three categories of assessment:
1) Symptoms
Patients report their experiences of neuropathy symptoms, such as:
👉 Numbness.
👉 Tingling or "pins and needles."
👉 Burning pain.
👉 Weakness.
👉 Unsteadiness while walking.
Scores are assigned based on the presence and distribution of these symptoms (e.g., feet, legs, hands).
2) Reflexes
The presence or absence of key reflexes, such as the Achilles tendon reflex, is evaluated.
Reflexes are graded (e.g., absent or reduced), with higher scores for more severe abnormalities.
3) Sensory Tests
Physical examinations assess specific sensory modalities, including:
👉 Pinprick sensation.
👉 Temperature perception.
👉 Vibration sense (usually tested with a tuning fork).
👉 Pressure sensation (often tested using a monofilament).
Deficits in these tests are scored according to severity and the number of affected sites.


Scoring
Each symptom, reflex abnormality, and sensory deficit is assigned a score. The total score ranges from 0 to 19, with higher scores indicating greater severity of neuropathy.
0–5: No or mild neuropathy.
6–8: Moderate neuropathy.
9–19: Severe neuropathy.
The Toronto Clinical Neuropathy Scoring System is a practical and reliable tool for diagnosing and grading neuropathy, facilitating timely intervention and ongoing management.
